Minor issues with recent FPBX upgrade from 4.3 -> 4.4 -> 4.5.11

Hope this is a simple issue that I'm having...
After following the instructions here to upgrade my FusionPBX version, I've noticed a couple of minor issues...
1. my Advanced > Upgrade page seems to be missing the "Source Code" upgrade option.​
2. my Advanced > Adminer page shows: "No input file specified"​
Not sure if anything else is broken, but phones seem to be working...

The server was initially setup in May 2016 on Debian Jessie base, with FusionPBX 4.3 & Freeswitch 1.6
Currently still on Jessie
FusionPBX 4.5.11
Switch 1.6.7 (64bit)

These are not actually bugs, they are new features that non-paying users generally do not get to know about. My understanding is adminer has been removed now, I do not know if its because of security or something else.

The source code thing is probably a permission, check in advanced/group manager. There have been quite a few permissions that for some reason have been removed by default from super administrator, this definitely won't be the only one.
