FusionPBX 5.2 Fax Coversheet Issues

Hi there,

I'm trying to figure this one out, but it looks like the coversheet isn't getting generated with the dates and field names...

IE:
Instead of
Date: (Todays Date)
To: (Fax Number)
From: (From Identity)

We get:
:
: (To Number)
: (From Information)

I've checked the fax_send.php and everything looks to be correct, but I can't figure out where the field names get generated. I've also tried replacing the fax_send.php with the source from Github but that's had no affect either.



Any help would be greatly appreciated.

One small update, this only appears to be affecting email to fax jobs, and not manually created jobs.
