when you log into the fusion web interface and try downloading the config file from the devices page, does it work? Do you see any errors in the logs of nginx?

Is this a new deployment? Is 'enabled' set to true in the Provisioning section of Default Settings?

your voicemail_id is set to S1001. Is that correct? I ask because your extension is 101.

Can you try deleting them both and then rereun menu defaults? Also, can you check your core/domains/app_menu.php file to make sure there isny any duplicates? Make sure that uuid and parent_uuid is only populated once.

Are you setting up a trunk in FreePBX to Fusion, or are you using a FreePBX extension? You should use a trunk on FreePBX

Do you have an inbound DID dedicated to fax? If so, when you direct that DID to an extension, are you able to complete that call?

Go to Advanced>Menu Manager and open the default (unless you are using a custom menu). Ctrl+F for "Domains" Delete one of them.

@Kenny Riley Thanks for the info! just to confirm, do you see the messages you send populating in the database? Do you see them in https://x.domain.com/app/sms/sms_mdr.php ? I am testing with Telnyx now. I was able to get inbound to go to GSWave and I'd be happy to troubleshoot with you.
