I'm running Ver 5.0.3,
I have several domains running on this system and the existing domains have no issue,
however I'm having issues creating new domains whereby the dialplan is not created properly (Dialplan > Dialplan Manager):
Any idea to what might be the problem? I have not installed anything new in particular lately.
The below screenshot shows how it should be on a normal system (ver 4.5.24, on my dev server):
