I found our trunk provider banned by fail2ban in the chain "f2b-fusionpbx-404", so calls was rejected.

Can someone explain what does it mean that chain? I cannot find specific documentation.

Off the top of my head....

Your carrier will be sending calls for a number either not configured or misconfigured and the number is not being matched.

This will cause a ban.
 
No, its perfectly sane, people dialling numbers not on your system are usually hack attempts.

If you do not want this behaviour you can disable the rule in the fail2ban config /etc/fail2ban/jail.local
 
I mean strange that my VoIP provider has sent wrong calls to my PBX...
However I whitelisted its IP and only it in fail2ban.
 
You might want to review your destinations to determine why you aren't answering the number that your trunk provider is routing to you. If it's an unallocated number, you might want to answer the call and simply play your own announcement for the unallocated number. You might want to check that your trunk provider doesn't charge a penalty for not properly answering calls they route to you.
