FusionPBX with dual ethernet

I'm struggling to get the FusionPBX working with 2 NICs.

1st NIC is setup for internal 192.168.10.x subnet and I have domain created for it.

2nd NIC is set with IP from range provided by SIP trunk company 10.0.23.x. 2nd NIC is straight into their equipment. They only support IP authentication. I've whitelisted there 172.46.30.x subnet to accept all the calls from it. Also added route to 172.46.x0.x via gateway of 10.0.23.x (provided by SIP trunk company)

I've changed in Variables ext_ip and rtp_ip to be the IP of 2nd NIC. However I'm not getting response to the invite. If I change local_ipv4 to be 10.0.23.x I can receive calls but unable to register extensions on 192.168.10.x subnet.

Has anyone got this kind of setup working and can point me in the right direction?
