IP Authentication not working

I have an old analog cisco router. This is used as a TDM handoff. The router is behind a NAT and the FusionPBX is in the cloud. The port forwading rules are in place so that FusionPBX can talk to the cisco router. I have created a bridge and inbound is working with two way audio. The problem is on the outbound side. I have created an extension and removed the password and under the advanced tab I put the public ip of the cisco router as well as the private IP. I see the call come in and then it dies. Any idea what I am missing?

Found my issue. I forgot to add the domain to the ACL. Once I did that issue was resolved.
