Provisioning issue

I've created a user and tied the mac address to it for provisioning but it can't find the file on the main domain account. I tested with a SIP-T29G as well as a web browser and I get a 404.

I've replaced sensitive data.

Is there a step I'm missing?

Have you actually enabled provisioning in the default settings?

Ya actually read through the readme again and turned it on. Now I get this error.

file not found /var/www/fusionpbx/resources/templates/provision///{$mac}.cfg

UPDATE
This is when I go to the extension, click on the provisioning and click files and select MAC.cfg
