We have had our system running fine for many months now. Every now and again we have an incoming call blocked because our provider has a very large range of IP addresses. We add the to the domains list in the ACL, remove them from fail2ban and voila, all is fine.

However, suddenly this seems to have stopped working. I have added the most recent addresses into the domains section of the ACL using CIDR and ensured it says allowed. Saved, restarted. But, the ip address still keeps getting blocked and the message below keeps appearing the logs.

Rejected by acl "domains". Falling back to Digest auth.

Is my list being ignored? ANy help will be greatly appreciated.

When you update your ACL is is good practice to flush cache and reload ACL from the SIP status page or using the FreeSWITCH CLI, whichever you prefer.
