Dont know what version your running but I fought this issue for some time. Ended up blowing away the server and building a new one on the updated branch and no issues since. I know thats not the answer you wanted but it did work. Claudio

All other notifications work i.e voicemail, missed calls, fax success or failure notifications. The only thing that isnt working is sending the fax via email as an attachment when it is received. I have checked the mail log in /tmp and it is not showing any error or attempt to send the email...
