What would cause this to get triggered?

I checked the user logs and do not see any failed login attempts. The logs show that the user logged in successfully.

I checked the user logs and do not see any failed login attempts. The logs show that the user logged in successfully.

It’s possible that fail2ban block the IP the user is trying to log into. I’d check if their IP is banned from multiple failed sip reg attempts.

As a safety measure, if an email address is associated with an extension , and then the email is later removed/changed, if there is an associated user login with that email, it suspends the account

I don't see a change in the email address; it's using the same one I set up. let me find out from the end user what they may have done.

Please confirm if their IP is in Fail2Ban.
