What would cause this to get triggered?

I checked the user logs and do not see any failed login attempts. The logs show that the user logged in successfully.
 
As a safety measure, if an email address is associated with an extension , and then the email is later removed/changed, if there is an associated user login with that email, it suspends the account

I don't see a change in the email address; it's using the same one I set up. let me find out from the end user what they may have done.
 
The IP is not listed in F2B.

I received more information: the user updated the email address on an extension that had his email listed. After changing the voicemail email address under the extension to the correct email, he was locked out shortly after.

The user confirmed that the email address was not changed on his profile, only on the extension email field.(This part I cannot verify, as no logs show it)
